18U All-Star Team Waiver

LIABILITY WAIVER

I, the undersigned, as the parent/legal guardian of the above-named participant, acknowledge and fully understand that participation in water polo involves inherent risks, including but not limited to bodily injury, drowning, and other accidents. I acknowledge that the Florida Nessies Youth All-Star Water Polo Team, its coaches, staff, volunteers, and affiliates assume no responsibility for injuries, accidents, or damages incurred during practices, games, tournaments, travel, or any related activities.

I hereby waive, release, and discharge the Florida Nessies, its officers, agents, sponsors, affiliates, and employees from any and all claims, liabilities, and causes of action arising from or related to my child’s participation in team activities. I agree to indemnify and hold harmless the Florida Nessies from any loss or liability resulting from any injury or damage my child may sustain while participating in the program.

I further acknowledge that my child is physically fit to participate and has no known medical conditions that would prevent safe participation. I agree to inform the coaching staff of any medical concerns, allergies, or injuries that could impact participation.

Medical Authorization: In the event of a medical emergency, I grant permission for the coaching staff or an authorized team representative to seek medical treatment for my child as deemed necessary. I understand that I am responsible for any medical expenses incurred.

PHOTO AND MEDIA RELEASE

I grant the Florida Nessies 18U All-Star Water Polo Team the irrevocable right to use, publish, and distribute photographs, videos, or other media featuring my child in promotional materials, including but not limited to team websites, social media, flyers, and advertisements. I understand that these images may be used without compensation and waive any right to inspect or approve the final product.

I acknowledge that my child’s participation in team activities may be recorded for promotional purposes, and I release the Florida Nessies from any claims related to the use of these materials.

I consent to the photo and media release.
